SPE - Parent Transporation Change Form

Thank you for using this form to submit daily transportation changes for your student!  This form is for DAILY changes only.  Permanent changes must be submitted as a handwritten note to the homeroom teacher. Voicemails, emails, and text will NOT be accepted to change a student's transportation.  All Transportation Changes must be entered by 1:00 p.m. on the day of the change.  Transportation changes for the following day cannot be entered until after 4:00 p.m.
